The American Optometric Association recommends optometrists refer patients suspected of having diabetes to a physicians for further evaluation and treatment. Optometrists must refer patients with undiagnosed diabetes who present with diabetic retinopathy during the initial examination for treatment of their diabetes.

Appropriate communication with the patient’s primary care physician is critical for proper coordination of the patient’s care. Due to the nature of diabetes, a multidisciplinary approach to patient management is essential. Optometrists should encourage patient to see a diabetes care provider to work toward achieving the goals for diabetes control.

The Diabetes Control and Complications Trial

The Diabetes Control and Complications Trial (DCCT) was a major clinical study conducted from 1983 to 1993 and funded by the National Institute of Diabetes and Digestive and Kidney Diseases. All DCCT participants were monitored for diabetic retinopathy. Study results showed that intensive therapy reduced the risk for developing retinopathy by 76 percent. In participants who had some eye damage at the beginning of the study, intensive management slowed the progression of the disease by 54 percent.
